Phase 4 Close and Sustain

Every project ends. The change shouldn't.

Phase 4 is where you prove the project delivered and make sure the change outlives the project team. You'll measure results against the charter, hand the solution to the people who'll run it day to day, and close out with lessons that make the next project smarter. Skip this phase and two things happen: nobody can say what the project achieved, and the change quietly slides back to the old way within months.

Who's involved 

You, your Sponsor, your project team, and the operations owners taking over.

By the end you'll have 

Measured results, a clean handoff to operations, captured lessons, and a formally closed project.

You're done with this phase when…

  • Results are measured against the charter's measures of success
  • Your Sponsor and stakeholders have seen the final results
  • The solution is handed off to operations with clear ownership
  • Reinforcement is in place so the change sticks after the project team disbands
  • Lessons learned are captured and shared
  • The project is formally closed and the team recognized
